Aspirin Use in Pregnancy

The American College of Obstetricians and Gynecologists (ACOG) recommends low-dose aspirin (81 mg daily) for pregnant women who are at high risk or moderate risk of developing preeclampsia.

High-Risk Group (One or More Risk Factors)

ACOG recommends low-dose aspirin starting between 12 and 28 weeks of gestation (preferably before 16 weeks) for women with one or more of the following high-risk factors:

  • History of preeclampsia, especially with adverse outcomes
  • Multifetal gestation (e.g., twins, triplets)
  • Chronic hypertension
  • Type 1 or Type 2 diabetes
  • Kidney disease
  • Autoimmune diseases (e.g., lupus, antiphospholipid syndrome)
Moderate-Risk Group (Two or More Risk Factors)

ACOG also suggests considering low-dose aspirin for women with two or more moderate-risk factors, including:

  • First pregnancy
  • Maternal age ≥35 years
  • Obesity (BMI ≥30 kg/m²)
  • Family history of preeclampsia (mother or sister affected)
  • Socioeconomic disadvantages or limited access to prenatal care
  • Personal history of low birth weight or previous pregnancy complications…
